The Manufacturers Descriptive Name Change Form on the TPA website has been updated and can be accessed at the following link: Drug Manufacturer Descriptive Name Form Please complete the form if there has been a change in the Descriptive Name of your organization. Manufacturers are instructed to provide the Descriptive Name exactly as it appears on your organization’s EFT transmission file. The TPA distributes this information to Part D sponsors in advance of each quarterly invoice to allow sponsors to more easily identify payment transactions originating from your organization.
